Salubrio offers comprehensive healthcare evaluations with the intention of providing the most complete evaluation and personalized treatments available. To provide this care, time and effort is required on the part of both patient and physician. In return, you will receive the greatest understanding of your body and health, and be most empowered to create the life and lifestyle that works best for you long-term.

To schedule a comprehensive naturopathic evaluation, please download and complete the following forms. Once returned to Dr. Koch (either via e-mail attachment or hard copy), she will schedule you for your first appointment.

Rescheduling

Kindly give at least 24 hours notice to reschedule an appointment.

Location

Salubrio is located 1 mile south of Central within “The Source for Creating Sacredness” at 1111 Carlisle Ave. SE. The Source is on the southwest corner of Carlisle and Anderson Ave.

Monday-Friday by appointment only.

Fees

The first visit for a comprehensive approach is 1.5 hours, includes the time I spend on reviewing the requested documents/health history, and costs $250.

For a more basic or urgent evaluation, we can schedule the first 1.5 hour appointment, cost is $175, and get started right away with treatment. The more comprehensive paperwork can be done at a later time if we deem it important to get a fuller picture. Follow-up visits of 1 hour are $100.

If you are interested in acupuncture only, rather than comprehensive care, a first visit is $80, follow-up visits are $60.

Gross receipts tax, lab fees, and supplements are at additional cost to all appointments.

Payment

Salubrio gladly accepts cash, check or MC/Visa for payments. We do not accept insurance of any kind. Please check with your insurance company if they will reimburse you for out-of-network services. We do provide all patients with a ‘superbill’ receipt for services rendered with diagnostic codes as required by insurance providers.
